I confirm that I am physically able and healthy to safely train in this activity and accept that my participation involves a risk of injury.

I agree to the school policy of zero tolerance for racism, sexism, or intolerance on the grounds of religion, sexuality, gender identity, immigration status, or lack thereof.

I acknowledge that this is a martial art taught for historical, sporting, and recreational purposes. I agree not to use any skills learned to threaten, intimidate, or harm others, nor to apply them unlawfully or maliciously.

Occasionally, we may take photos and videos during class for educational and promotional purposes. I understand that if I don’t want to appear, I will tell the instructor and move out of shot.
